Behavioral health credentialing for UnitedHealthcare members runs through Optum Behavioral Health, which works with providers on its Provider Express site. Therapists apply online after registering on Provider Express, Optum reads the application data from CAQH ProView in most states, and Optum's credentialing plan sets out exactly who qualifies and what each application needs. This page follows Optum's own published pages and its current credentialing plan.
Pick the right path on Provider Express
Optum's Our Network page splits applicants into five groups:
- Individually credentialed clinicians: you must be a solo clinician, or practice in a group that does not have a group agreement with Optum.
- Group with individually credentialed providers: your group already has an Optum group agreement, and each clinician is credentialed individually under it. Ask your group administrator first.
- Group with agency credentialed providers: for organizations licensed or certified at the organizational level, such as community mental health centers, FQHCs, rural health clinics and opioid treatment programs.
- Facility or hospital-based programs offering inpatient, residential, partial hospitalization or intensive outpatient care.
- Autism, ABA and BCBA providers, recruited for Optum's ABA network.
Individual clinicians then register or log in to Provider Express and use its Join Our Network feature to reach the credentialing application.
Can a group get its own contract?
Optum's group page says group contracts are available only under limited circumstances. Among its criteria: every eligible clinician agrees to participate, claims go out under the practice tax ID with the individual clinician identified, the practice has central intake and central billing, it can cover and triage urgent appointments, and at least 5 providers will be credentialed. Multi-disciplinary practices are preferred. A smaller therapy group usually credentials each clinician individually instead.
Who qualifies
Optum's 2026 to 2027 Credentialing Plan, United Behavioral Health and its Affiliates says applicants must be licensed to practice independently, without supervision or oversight. The plan's exceptions are limited to master's-level psychiatric clinical nurse specialists, physician assistants and behavior analysts, plus any provider types a health plan or state regulation requires Optum to allow. Associate-level therapists are not credentialed on their own.
The plan also says Optum first makes a preliminary decision that it wants to contract with an applicant before credentialing starts. In some markets Optum limits additions; its network page, for example, has announced limits on its Arizona Medicaid network while keeping commercial and Medicare Advantage networks open to behavioral health providers.

Claim Your Free CredentialingWhat the application needs
The credentialing plan says Optum uses the CAQH application unless a state requires otherwise, and requires with each application:
- Professional liability insurance at the limits the plan sets for physicians and for other clinicians, or as state law specifies.
- A 5-year work history in month and year, with explanations for gaps of 6 months or more.
- For prescribers, a current DEA and state controlled-substance certificate for each state where they prescribe.
- A W-9, and proof of meeting Medicare and Medicaid participation requirements.
- Disclosures on licenses, discipline, convictions and privileges, and a signed attestation and release.
The mistakes Optum says slow applications down
Optum's individual clinician page lists the most common problems found in its own audit of credentialing applications:
- A CAQH profile that is incomplete or expired, or that Optum is not authorized to read. In CAQH, the authorization should include United Behavioral Health / U.S. Behavioral Health Plan.
- Group details or practice locations on the Optum participation form that do not match CAQH.
- Out-of-date CAQH details: practice information, credentialing contact, license and liability insurance dates.
- The wrong document attached, an unsigned W-9, or an incorrect tax ID.
- Slow return of DocuSign documents such as the individual contract and disclosure of ownership.
Every item is fixable before you apply. Our CAQH ProView help page covers the profile, authorization and attestation.
Checking status and staying credentialed
Individual providers track the application in Provider Express under My Network Status, using Check Initial Credentialing Status. Groups, agencies and facilities contact Optum Network Management at (877) 614-0484. After approval, Optum reviews participating clinicians for continued participation every 36 months, and CAQH still needs re-attesting every 120 days (every 180 days in Illinois), per the CAQH quick reference guide.
