CAQH ProView is the free online profile most commercial health plans read when they credential a provider. Set it up once, authorize the plans you are applying to, attest it, and then keep it current with a re-attestation every 120 days (every 180 days in Illinois). Most stalled commercial applications we see trace back to one of four CAQH problems: the profile was never attested, a plan was never authorized, a document expired, or the re-attestation lapsed. This page covers each step and how to avoid those four.
What CAQH ProView is, and what it is not
CAQH ProView replaces the paper credentialing application. You enter your professional data once and the organizations you authorize can read it, so you are not filling in a separate packet for every plan. CAQH describes it as free to providers and available in every state.
It is not an application to any plan. Completing CAQH does not put you in network. You still apply to each plan, the plan pulls your CAQH data during its review, and you are in network only once the plan's contract is signed and the effective date is confirmed. A complete CAQH profile simply means the plan has nothing to wait for on your side.
What to gather before you start
The CAQH ProView Provider Quick Reference Guide lists what a new user needs. In practice that is:
- Your CAQH Provider ID, from a plan's invitation letter or by self-registering in the CAQH ProView portal.
- Your identifiers: your individual NPI from NPPES and any Medicare and Medicaid numbers.
- Every current and previous practice location.
- Scanned copies of your CV, license, DEA and state controlled-substance certificates if you prescribe, IRS Form W-9, and malpractice insurance face sheet.
- A summary of any pending or settled malpractice cases.
- A previously completed credentialing application, if you have one, to copy answers from.
CAQH's guide suggests allowing about two hours for a first profile, and you can save and come back across several sessions.
Setting up the profile, step by step
- Register. Enter your CAQH Provider ID and your identity details, then create your login and security questions.
- Complete every section. Under the profile's data entry area, work through each section and save as you go; CAQH warns that closing the browser without saving loses the page.
- Review and fix. The review step lists required fixes (which block completion) and suggested fixes (answers that look inconsistent). Clear both. It also shows missing and expired documents.
- Upload documents for every item flagged as needed.
- Authorize access. Choose either every organization that lists you as an affiliated or applying provider, or only the organizations you name. If a plan you are applying to does not appear, CAQH's guide says to ask that plan to add you to its roster.
- Attest. Review the summary and attest that it is true, correct and complete. Until you attest, plans cannot rely on the profile.

CAQH sends a reminder to re-attest every 120 days, or every 180 days for providers practicing in Illinois. Re-attesting means logging in, reviewing your data, updating anything that changed, uploading new documents, and attesting again. If the date passes, plans see a profile that is out of date, and applications in progress can stop. Renewals of your license, malpractice policy and DEA registration also need a fresh upload each time.
Update CAQH whenever something changes, not only at re-attestation: a new practice location, a new group, a name change, or a new license in another state for telehealth clients.
The four CAQH problems that stall credentialing
- Profile complete but never attested. The plan sees an incomplete profile and waits.
- Plan not authorized. If you chose to name organizations one by one and missed a plan, it cannot read your data.
- Expired document. A malpractice face sheet or license that expired after you uploaded it.
- Missed re-attestation. The 120-day reminder went to an inbox nobody checks.
Two more that are less obvious: a practice address or name that differs slightly from the one on your NPI record, and a CV with unexplained gaps in work history. Both prompt questions from the plan's credentialing team.
Group practices: one profile per clinician
CAQH profiles belong to individual providers, not the practice. A group with several therapists or physicians manages several profiles, each with its own re-attestation date. Keep a simple calendar of every clinician's date, and give your billing or credentialing partner the access the clinician authorizes, so updates do not wait on one person.
