This calculator estimates how much revenue a practice loses each month to denied claims that are never reworked or appealed. You enter your own numbers; nothing is prefilled, because no published average fits every specialty and payer mix. The formula is shown below so you can check the arithmetic yourself.

The formula
Monthly revenue not collected = claims per month × average paid amount per claim × (denial rate ÷ 100) × (share of denials never reworked ÷ 100). The yearly figure multiplies the monthly figure by 12.
It is deliberately simple. It assumes a denied claim would have paid about the same as your average paid claim if it had been fixed, and it ignores denials that are reworked late and paid at a lower amount. Treat the result as a size of the problem, not a forecast.
Where to find your inputs
- Claims per month: your billing system's claim submission report for last month.
- Average paid amount: total insurance payments for the month divided by the number of claims paid.
- Denial rate: claims denied on first submission divided by claims submitted. Count denials, not rejections that never reached the payer.
- Share never reworked: of last quarter's denials, how many were never corrected, resubmitted or appealed. If nobody tracks this, that is your first finding.

30-Day Free TrialWhat usually causes the leak
- Eligibility and coverage errors that are cheap to prevent and slow to fix. See eligibility verification.
- Missing or expired prior authorizations. See prior authorization denied.
- Coding and modifier errors that a scrubber should catch before submission.
- Filing limits passing while claims wait. See timely filing limits by payer.
- No one owning denials. If every denial is not worked by reason code, the second half of the formula grows. See denial management services and the denial reason codes in the X12 claim adjustment reason code list.
How to shrink both numbers
Lower the denial rate by checking eligibility, authorizations and coding before claims go out, and lower the never-reworked share by working every denial within days. Our guide to why insurance claims are denied covers the common causes, and the 2026 Medical Billing Benchmark Report lists published benchmarks to compare against.
