Behavioral health billing in New Jersey changed on January 1, 2025, when NJ FamilyCare moved outpatient mental health and substance use services into managed care. Counseling and psychotherapy for NJ FamilyCare members that used to be billed fee-for-service now goes through the member's managed care organization (MCO), and the MCOs are ending their transition-period protections one by one. For a New Jersey therapy practice, the order of work is now: enroll with NJ FamilyCare, credential and contract with the MCOs, and follow each MCO's authorization rules. This page sets out what the state has published.
Behavioral health integration: what moved and when
The state's NJ FamilyCare Behavioral Health Integration Updates (March 12, 2026) describes a phased approach. Phase 1 went live January 1, 2025 and covers outpatient behavioral health services for adults and children, including:
- Mental health outpatient counseling and psychotherapy.
- Mental health partial hospitalization, partial care, and outpatient hospital or clinic services.
- Substance use outpatient counseling, intensive outpatient and outpatient clinic services, including ambulatory withdrawal management, peer support and care management, and substance use partial care.
Phase 2, covering residential services and opioid treatment programs, has been delayed to go live in 2027, and the scope and timing of later phases are still to be decided.
The transition period is ending, one MCO at a time
To avoid disrupting care, the state set transition-period policies during Phase 1. The March 2026 update says the Division of Medical Assistance and Health Services (DMAHS) began ending them on November 1, 2025, MCO by MCO, as each passes a readiness review:
- Aetna ended its Phase 1 transition policies on November 1, 2025.
- Horizon NJ Health planned to end its transition period on April 1, 2026. For services starting on or after that date, Horizon applies medical necessity review to prior authorization requests and may deny them; from July 1, 2026, out-of-network providers may be paid below the fee-for-service rate floor.
- Fidelis Care, UnitedHealthcare and Wellpoint continue transition policies until further notice, with advance notice from the state and the MCO before each one ends.
The practical lesson is in the state's own advice: credential and contract with all five MCOs. In network, a provider is guaranteed fee-for-service rates rather than single case agreement rates, and the prior authorization process is simpler. Out of network, requirements vary by MCO.
Who can bill: licensed therapists and NJ FamilyCare enrollment
New Jersey law P.L.2023, c.213, approved January 8, 2024, provides for NJ FamilyCare and its contracted MCOs to accept and pay claims from licensed clinical social workers, licensed professional counselors and licensed marriage and family therapists who hold a current license and are approved NJ FamilyCare providers, for covered services. Enrollment applications are published on NJMMIS by provider type. Licenses are issued by the State Board of Social Work Examiners, the Professional Counselor Examiners Committee, the Marriage and Family Therapy Board and the State Board of Psychological Examiners.
Because almost all outpatient therapy for NJ FamilyCare members is now paid by MCOs, state enrollment alone is not enough. Each MCO credentials and contracts separately, usually through CAQH. Our guide to CAQH ProView covers the profile the MCOs read.

The state's update tracks prior authorization requirements by MCO and by service, and warns that claims are denied for providers who do not follow them. Partial care, partial hospitalization and intensive outpatient services generally need authorization. Requirements for outpatient counseling and psychotherapy differ by MCO and by network status, so we confirm the current rule with each client's plan before the first session and track authorizations as they run. Our prior authorization services cover the requests and follow-up.
Commercial plans in New Jersey
For commercial and state-regulated plans, New Jersey's prompt pay law sets payment deadlines and appeal rights. Those rules, and Medicaid enrollment for other specialties, are on our page for New Jersey medical billing. NJ FamilyCare's health plan list shows which MCOs serve the state.
