Behavioral health billing in Illinois depends on which license the clinician holds and when Illinois Medicaid began paying for it. The Department of Healthcare and Family Services (HFS) began fee-for-service payment for licensed clinical social workers and licensed clinical psychologists from January 1, 2017, and for licensed clinical professional counselors and licensed marriage and family therapists from December 1, 2021. Each must be enrolled in IMPACT, the state's provider enrollment system, under the right provider type, and most Medicaid members are in HealthChoice Illinois managed care plans with their own billing rules. This page covers what HFS has published for Illinois therapy practices.
Which licenses Illinois Medicaid pays
| License | Paid from | HFS notice |
|---|---|---|
| Licensed Clinical Social Worker (LCSW) | Dates of service on or after January 1, 2017 | December 28, 2016 |
| Licensed Clinical Psychologist (LCP) | Dates of service on or after January 1, 2017 | December 28, 2016 |
| Licensed Clinical Professional Counselor (LCPC) | Dates of service on or after December 1, 2021 | November 30, 2021 |
| Licensed Marriage and Family Therapist (LMFT) | Dates of service on or after December 1, 2021 | November 30, 2021 |
Licenses are issued by the Illinois Department of Financial and Professional Regulation. The 2021 notice ties enrollment to the license: an LCPC must be licensed under the Professional Counselor and Clinical Professional Counselor Licensing and Practice Act, and an LMFT under the Marriage and Family Therapy Licensing Act. The notices cover the clinical licenses; a counselor or therapist without one of them is not listed as an enrolling provider.
Enrolling in IMPACT under the right type
Enrollment is through IMPACT. The 2021 notice gives the exact selections for counselors and family therapists:
- LCPC: provider type Behavior Health Services, specialty Licensed Clinical Professional Counseling, no subspecialty.
- LMFT: provider type Behavior Health Services, specialty Licensed Marriage and Family Therapist (LMFT) Services, no subspecialty.
Choosing the wrong type or specialty is a common cause of claims that do not pay even though the clinician is "enrolled." We check the IMPACT record against the clinician's license before the first claim.
What each license can bill
- LCSWs: diagnostic evaluation and therapeutic services in the office, home or community for individual, family or group psychotherapy whose purpose is treating the patient. The 2016 notice says this fee-for-service payment applies to LCSWs practicing outside a community mental health center, FQHC or rural health clinic.
- LCPs: psychological and neuropsychological testing, diagnostic evaluation and the same psychotherapy services.
- LCPCs and LMFTs: certain diagnostic and therapeutic services in the office, home or community, consistent with the services and rates on HFS's Fee Schedule for Licensed Practitioners of the Healing Arts, plus a limited set of community-based mental health services under 89 Ill. Admin. Code 140.453 (the Group A services in HFS's community-based behavioral services handbook).
Group psychotherapy must meet the guidelines in 89 Ill. Adm. Code 140.462. HFS does not pay for services listed in 89 Ill. Adm. Code 140.6, for self-administered or self-scored tests of cognitive function, or for biofeedback therapy under the 2016 notice.

30-Day Free TrialHealthChoice Illinois managed care
Both notices send providers to the member's HealthChoice Illinois managed care plan for detailed billing instructions when the client is enrolled in managed care. In practice that means two steps for most Illinois practices: IMPACT enrollment with HFS, then credentialing and a contract with each plan your clients use. Each plan applies its own authorization and claim rules, so we verify the plan at every visit.
Illinois has its own CAQH timing
Most commercial plans and many Medicaid plans credential through CAQH ProView. Illinois providers re-attest every 180 days rather than every 120 days, according to the CAQH quick reference guide. It is a small difference, but practices that move clinicians between states, or use a national credentialing calendar, can get it wrong. Our CAQH ProView help page covers setup and re-attestation.
What we do for Illinois behavioral health practices

- Credential and contract with the HealthChoice Illinois plans and commercial plans your clients use.
