Psychological testing is billed in pieces that are easy to get out of order. CPT 96130 and 96131 carry the professional thinking: interpreting results, integrating them with everything else known about the patient, writing the report and giving feedback. The hours spent actually administering tests go elsewhere. Most testing denials come from blurring those two, or from billing a multi-day evaluation as if every day were a new one.
What CPT 96130 and 96131 describe
96130 is psychological testing evaluation services by a physician or other qualified health care professional, first hour. 96131 is the same service, each additional hour, and is an add-on reported only with 96130. The descriptor lists what the evaluation hour includes:
- integration of patient data,
- interpretation of standardized test results and clinical data,
- clinical decision making,
- treatment planning and the report,
- interactive feedback to the patient, family members or caregivers, when performed.
What it does not include is the time spent giving and scoring the tests. That is reported with the test administration codes: 96136 and 96137 when the psychologist or other qualified professional administers, and 96138 and 96139 when a technician does. The APA Services 2026 billing guide is explicit that administration and scoring codes are always reported together with the professional evaluation codes, on the same or different days.
CPT 96130 vs 96132
The two families share an identical structure, a first-hour code plus an add-on per additional hour, and differ in what is being evaluated. 96130 and 96131 are psychological testing evaluation: mood and emotional functioning, personality, psychopathology and similar domains. 96132 and 96133 are neuropsychological testing evaluation: cognitive functioning in relation to brain health, for example memory, attention, language, executive and visual-spatial abilities.
The choice follows the referral question and the instruments used, not the credential of the person billing. The Medicare edit file contains no procedure-to-procedure pair between 96130 and 96132, but billing both for one patient still requires two genuinely distinct evaluations, each with its own time.
Time rules and the episode of care
These are timed codes, and CPT's general time convention applies: a unit of time is reached once the midpoint has been passed. In practice the first hour of 96130 needs at least 31 minutes of evaluation work, and each 96131 hour is reported once a further 31 minutes or more has accrued beyond the previous full hour.
The bigger trap is the calendar. Testing evaluations often span several days: an interview, one or more testing sessions, scoring, report writing and a feedback session. The APA guide sets out how that should be billed:
- One base code per episode. 96130 is used once for the whole evaluation, even when the work spans additional days. Later hours on any day are 96131.
- Bill at the end. The total time is reported when the episode is complete, with base and add-on codes listed against their actual dates of service on a single bill.
- The episode ends with feedback. It concludes when the report is complete and interactive feedback has been provided.
- Long evaluations need a reason. Where an evaluation exceeds eight hours, not counting feedback, the medical necessity for the extended time should be documented.
Interactive feedback is billed under the evaluation codes, not as a separate psychotherapy visit, and the APA guide notes it can be provided by telehealth or audio only.
Documentation requirements
- The referral question and why testing is medically necessary to answer it.
- Instruments administered, by name, with who administered each one and the time taken, so administration time and evaluation time are clearly separated.
- Evaluation time by date, covering interpretation, integration, report preparation and feedback.
- The written report, integrating results with history and other clinical data, with diagnostic conclusions and recommendations.
- Feedback: date, who received it and how it was delivered.
- Technician supervision where 96138 or 96139 were used, since interpretation remains the psychologist's responsibility.
Common denials and the exact fix
96130 billed on more than one date for one evaluation
The first-hour code was repeated on day two or day three of the same episode. Each date passes Medicare's one-per-day unit edit on its own, but on review the second base code is denied or recouped.
Fix: track the episode, not the visit. Report 96130 once, everything after it as 96131, and submit when the feedback session closes the episode.
Test administration time counted as evaluation time
Hours spent giving tests were folded into 96130 and 96131, which overstates the evaluation and leaves 96136 to 96139 unbilled or duplicated.
Fix: log administration minutes and evaluation minutes separately from the first session, and code each to its own family.
Time duplicated with the diagnostic interview
NCCI requires the diagnostic evaluation, test administration and testing evaluation to be distinct services when they share a date, and bars reporting time for duplicated information gathering or interpretation.
Fix: when 90791 opens the episode, keep the interview time out of the testing evaluation total and say so in the note.
Interactive complexity added to the evaluation
90785 sits under 96130 and 96131 at indicator 0 in the Medicare edit file. It is denied every time it appears beside them.
Fix: remove 90785 from testing claims entirely and document communication factors in the narrative instead.
Screening instruments billed as testing
NCCI states that these testing codes cannot be reported for a self-administered test, or for tests reportable as part of an E/M service. A brief rating scale scored in a few minutes is not a testing evaluation.
Fix: report brief standardized screening with 96127 where it applies, and reserve 96130 for evaluations that meet the descriptor.
Modifier rules
- 59, XE, XP, XS and XU: an E/M service on the same date sits over 96130 at indicator 1, so a distinct E/M can be reported with the modifier on 96130 and a record that shows separate work. Modifier XE also appears on testing claims where psychologist administration (96136) and technician administration (96138) took place at separate encounters on one date.
- 95 and 93: 96130 to 96139 are on the CY 2026 Medicare telehealth list. Mark remote portions according to the technology used and the payer's instruction.
- HO and HP: some Medicaid programmes price testing by the evaluator's degree level.
NCCI bundling edits
From the Medicare practitioner procedure-to-procedure file effective October 1, 2026.
- 96130 and 96131 over 90785: indicator 0.
- 96130 and 96131 over 96127: indicator 0, misuse of the column two code.
- 96130 and 96131 over the adaptive behavior codes 97151 to 97158: indicator 0.
- 96020 over 96130 and 96131: indicator 0.
- 99483 over 96130: indicator 0. A cognitive assessment and care planning visit absorbs a same-day testing evaluation.
- Office, hospital, nursing facility and home E/M codes over 96130: indicator 1, standards of medical and surgical practice.
- 96130 and 96131 over 96110, 96113, 96146 and G0451: indicator 1, mutually exclusive; 96112 over both at indicator 1.
- 96156, 96158 and 96159 over 96130: indicator 1.
- No pair between 96130 and 90791, the test administration codes 96136 to 96139, or the psychotherapy codes. Those combinations are governed by the distinct-service and no-duplicated-time rules above, not by an edit.
Local coverage: is there an LCD for this code?
There is no national coverage determination for psychotherapy or psychiatric evaluation, so coverage is set locally and it differs by jurisdiction. Several Medicare Administrative Contractors publish a local coverage determination for psychiatric services together with a billing and coding article that carries the actual code list. Novitas Solutions, for example, maintains LCD L35101 (Psychiatric Codes, effective January 1, 2024) with billing and coding article A57130, which is where the covered CPT and HCPCS codes and the covered diagnoses actually live.
Two practical consequences. First, the diagnosis list that supports CPT 96130 in one jurisdiction may not support it in another, so the article to read is the one from the contractor that processes your claims. Second, LCDs and their articles are revised without much notice, which is why a claim that paid last quarter can deny this quarter with nothing changed on your side. Search the CMS Medicare Coverage Database by code and by your contractor, and re-check it when a denial pattern appears.
What CPT 96130 and 96131 pay
An evaluation is paid as one first hour plus however many additional hours the documented time supports, so the value of a testing episode depends on getting the hour count right. Medicare publishes relative values for each code. Your locality's geographic indices and the conversion factor below turn those into a payment. Where a plan requires prior authorization for testing, check whether the approval is stated in hours or units, because the claim has to match it.

| Global (no modifier) | 2.56 | 1.06 | 0.33 | 0.09 | 3.71 | 2.98 |
Source: CMS CY 2026 PFS Relative Value File, RVU26D (October release, published 08/26/2026). RVUs are national and unadjusted; commercial and Medicaid payers set their own rates and their own coverage.
In the 2026 CMS relative value file, CPT 96130 carries 2.56 work RVUs, with 3.71 total RVUs in a non-facility setting and 2.98 in a facility. The gap is practice expense: in an office the practice carries it, in a facility the facility does. Its PC/TC indicator is 0: CMS treats it as a physician service, so modifiers 26 and TC do not apply to it in any setting. Its global indicator is XXX: the surgical global period concept does not apply, so there is no postoperative period bundling other services into it. Across the psychological and neuropsychological evaluation codes, work RVUs run from 1.96 (96131) to 2.56 (96132); 96130 sits at 2.56.
Units per day. Medicare's medically unlikely edit allows 1 unit of CPT 96130 per patient per day (a date of service edit set by policy, rationale: code descriptor / CPT instruction). A second unit on the same date will not pay. MUE values are published quarterly in the Medicare NCCI practitioner services MUE table; the value above is from the table effective October 1, 2026.
To turn RVUs into payment, each component is multiplied by your locality's geographic practice cost index, the three are added, and the sum is multiplied by the 2026 conversion factor: $33.4009, or $33.5675 for qualifying APM participants (CMS-1832-F; CMS fact sheet). The CMS Physician Fee Schedule Look-Up Tool runs that calculation for your own locality.
2026 Medicare RVUs for CPT 96131
| CPT 96131: 2026 Medicare RVUs (national, before GPCI) | ||||||
|---|---|---|---|---|---|---|
| Line | Work | PE, non-facility | PE, facility | Malpractice | Total, non-facility | Total, facility |
| Global (no modifier) | 1.96 | 0.62 | 0.13 | 0.01 | 2.59 | 2.10 |
In the 2026 CMS relative value file, CPT 96131 carries 1.96 work RVUs, with 2.59 total RVUs in a non-facility setting and 2.10 in a facility. The gap is practice expense: in an office the practice carries it, in a facility the facility does. Its PC/TC indicator is 0: CMS treats it as a physician service, so modifiers 26 and TC do not apply to it in any setting. Its global indicator is ZZZ: it is an add-on code, always reported alongside its primary service and never on its own. Across the psychological and neuropsychological evaluation codes, work RVUs run from 1.96 (96131) to 2.56 (96132), so 96131 carries the lowest physician work in its group.
Units per day. Medicare's medically unlikely edit allows up to 7 units of CPT 96131 per patient per day (a date of service edit set on clinical grounds, rationale: nature of service/procedure). MUE values are published quarterly in the Medicare NCCI practitioner services MUE table; the value above is from the table effective October 1, 2026.
